

Health Center
Sheboygan Area Community Clinics, Inc. (SACCI) is a private, non-profit 501(c)(3) seeking funding to provide Sheboygan County’s most needy individuals and families with healthcare through the proposed Lakeshore Community Health Center.
Grand Opening 2011
The health center is projected to open early in 2011. It will be staffed by a nurse practitioner and three support staff. The center will be open four days a week and will provide comprehensive primary health care services and referrals to specialty care.
The top choice location for the clinic has been identified and most of the startup equipment needed has been donated and is in storage.
Healthcare Services Provided
Once running, the center will be able to provide 3,324 annual visits for primary and preventative healthcare including:
* Chronic disease management
* Medication monitoring
* Vaccine referrals
* School and Work physical exams
* Lab and X-ray referrals to hospitals
* Completion of rapid tests
* Health promotion and disease prevention
* Same day appointments for acute illness
Program Expansion
Future plans would expand the programs to include dental, behavioral health and case management services. The center applied to become a Federally Qualified Health Center in the fall of 2010 and is actively establishing partnerships with social service agencies in the area. Meetings to secure formal support of the center by major Sheboygan health providers are being held and additional funds are also being sought from the community.
