Report Envelopes
The form should be printed, filled out and securely attached to your campaign envelope. All yellow copies of pledge forms and all cash and check donations should be placed in the envelope. Do not forget to include your organization's name and campaign coordinator.
Completed Envelopes
When you have completed your company campaign collections, please contact the United Way office at (920) 458-3425 for envelope pick up. We ask that you contact us within two weeks of finishing your campaign.
